Start-of-flow trigger for a FAILED entry. Fires when an access_entry event is denied. Same out-pins as access granted, with granted=false. Use for security alerting: an unknown card at 3am is worth a notification.

A refusal is the more interesting half of an access system. access denied fires when a reader rejects something, and the method pin says what was presented. One refusal is somebody holding the wrong card; a run of them at three in the morning is worth waking a person for, which is why this trigger belongs on a notification path rather than only in a log file.
